They estimated 75-94% of agentic Responses API traffic was monitorable, and found gaps such as unmonitored compaction events, RL training runs, and headers that disable monitoring. OpenAI says all production issues are patched; METR lists five design ideals for monitors.

An opinion post from 21 October 2026 arguing that a widely feared PHP vulnerability has no easy exploit. The author says the many GitHub "PoC" repositories fail against a vanilla PHP instance in Docker. They claim LLM agents produced them by depending on a local-file-inclusion flaw or by modifying the server to be vulnerable. The author also argues that LLM defaults make many people's judgements identical.
